If you experience problems with the eForms web site, you can contact the USDA, ITS Service Desk Support Center by calling 800-457-3642 and then press option 2.

Please be prepared to supply the following information:

Your name.
The State and County where you live.
Your phone number, including area code and extension number.
A brief description of the problem.

The hours of operation are 6:00 AM CST - 6:00 PM CST, Monday through Friday. If you call after the normal hours of operation, you will be able to leave a voice mail message.

Local Service Center

Your local Service Center office will gladly answer all your questions about what programs are available and how to apply, what forms to use and how to complete them, and all other program related questions. The USDA Service Center Locator web site will provide contact information for the nearest service center in your area.

You may also want to consult the USDA web site which may be able to provide information about programs with which you may be interested in participating.
